Community Center Membership Packages and Daily
Admissions
I. OBJECTIVE Define the classification for the
general public, visitors of Madison, visitors of Dakota
State University and/or Dakota State University students
to access The Community Center.
II. CONTENT Per the Cooperative Use Agreement between
the City of Madison and Dakota State University, The
Community Center is available to the general public
through membership fees or daily guest passes. DSU
students carrying a minimum of six (6) credit hours per
semester will be members of The Community Center through
student fees. I
II. MEMBERSHIP AGREEMENT The classification of
Members, the amount of dues payable be members of each
class, the amount of fees, the suspension and expulsion
of Members, and all other matters affecting and relating
to the Members or Membership shall be under complete
control of The Community Center. Membership rates are
subject to change without notification. All memberships
are annual memberships.
It shall be policy of The Community Center to accept
applications for membership from an individual, couple,
family or business of good character with out regard to
race, color, creed, sex, national origin, or physical
disabilities.
The obligation to pay dues is not dependent on the
availability of The Community Center’s facilities.
Tournaments, repairs and (or) maintenance of the
facilities may make it necessary for The Community
Center to restrict use of one or more facilities or
temporarily close The Community Center that will not
reduce or suspend the Members’ obligation for payment of
dues.
IV. MEMBERSHIP PACKAGES AND DAILY ADMISSION
CLASSIFICATION Anyone under the age of two (2) doesn’t
need a membership or daily fee to enter The Community
Center, but they must be accompanied by a paying adult.
Admittance to the family center and/or front lobby is at
no cost. All other areas in the facility require a
membership or a daily pass to entire, unless enrolled in
special program or event. “Guests
requesting not to utilize the facilities, but wish to
watch in the pool area may do so without payment. Guests
must ask approval from front desk before entering into
facility.”
A daily pass allows entrance to The Community Center
for the entire day. Guest(s) has all the same membership
privileges as a Community Center member within The
Community Center.
A. Family Membership includes any member(s)
and their legal children who live under the same
household or is claimed as dependent. College students
can be included in a family membership as long as they
are claimed as dependent and have the same permanent
address. No fee to add a person on to a family
membership.
B. Adult Couple Membership is defined as a
primary member and their “significant other” who are
living at the same household address. Neither member can
claim the other member as dependent on a government
form.
C. Senior Couple Membership is defined as a
primary member and their “significant other” who are
living at the same household address. Neither member can
claim the other member as dependent on a government
form. One member must be 65 years of age or older.
D. Adult Membership is defined as an
individual member who is the age of 22 or older.
E. Senior Citizen is defined as an individual
member who is the age of 65 or older
F. Young Adult is defined as an individual
member who is or in between the ages of 19 and 21.
G. Walking Membership is defined as an
individual who is 14 years of age or older and has
access to the walking track only. An individual on a
walking membership must purchase a punch card for group
fitness classes.
H. High School Membership is defined as an
individual who is or will be enrolled in grade 9 thru
12. A legal guardian signature is required on the
membership application and agreement.
I. Middle School Membership is defined as an
individual who is or will be enrolled in grade 6 thru 8.
A legal guardian signature is required on the membership
application and agreement.
J. Elementary/Pre-School Membership is defined
as an individual who is older then the age of 2 and not
enrolled in grade 6. A legal guardian signature is
required on the membership application and agreement.
V. TEMPORARILY MEMBERSHIP PACKAGES The Community
Center acknowledges the use of temporarily membership
packages to be used for special promotions, events or
situations. Temporarily membership users have same
privileges as other membership packages unless otherwise
defined.
A. Summer/College Membership allows full
access to The Community Center during the contract
period of ninety days for $100. After the contract
expires the member has 30 days to apply the $100 towards
payment of an annual membership. Membership package
doesn’t allow access to the Madison Outdoor Pool.
VI. MEMBERSHIP DISCOUNTS The Community Center
acknowledges the use of discounts on membership packages
to be used for special promotions, events or situations.
Membership discounts are valid for one year unless
otherwise defined.
A. Corporate Membership Discounts are
available to a business with 5 or more employees
enrolled in a membership at The Community Center.
For up to 24% of employees: 5%
For 25-49% of employees: 10%
For 50-100% of employees: 15%
B. Surrounding Area Membership Discount is
defined as members living within any zip code located
outside of Lake County. Members are eligible for a 20%
discount on an annual membership. Proof of permanent
residency must be provided.
VII. PUNCH CARDS Punch cards are a pre-payment form
of daily admissions. Punch cards are non-refundable
after use.
A. Walking Track Visitor Pass allows
individual access to the walking track.
B. Visitor Pass allows individual access to
The Community Center for the day.
VIII. MEMBERSHIP CANCELLATION / REFUND POLICY If an
individual is unable to continue their membership by
reason of death, permanent disability, or residence is
permanently relocated outside of the county, the member
will be relieved from the obligations of this contract.
Legitimate verification of the circumstance and
cancellation must be presented in writing. If the member
has prepaid, the pro-rated amount will be refunded.
Downgrading a membership is not allowed.
The Community Center reserves the right at any time
to terminate the membership of any member for failure to
comply with any of the rules and regulations adopted by
The Community Center governing board. The terminated
member will be required to return his/her membership
card to The Community Center. The terminated member will
remain liable for the remainder of their contract
period. Any member who is thirty (30) days late with
payment to The Community Center may be terminated at the
discretion of The Community Center. A $10 service charge
is accessed for a check or bank draft that is returned
insufficient.
IX. MEMBERSHIP RENEWAL POLICY Members who have
selected a monthly bank debit will automatically have
their annual membership renewed at the current
membership rate, unless notification in writing is
received. To renew a paid-in-full membership, payment
will need to be received before the contract expiration
date. A $25 service charge will be assessed upon renewal
for membership contracts that previously cancelled due
to moving out of the county.
X. SCHOLARSHIP & FINANCIAL ASSISTANCE Financial
assistance is available, through the Madison Community
Foundation, to anyone who applies for assistance and
meets standard eligibility requirements. All members
utilizing the scholarship and financial assistance must
re-apply for financial assistance upon renewal of their
membership. Appropriate income verification needs to be
submitted with the scholarship application.
XI. ADDITIONAL MEMBERSHIP AGREEMENT TERMS A. CHECKING
IN: No member will be admitted to The Community Center
without checking in or registering at the front desk of
The Community Center. An adult must accompany children
under the age of six (6) at all times, unless the child
is enrolled in a program. Children under the age of
twelve (12) will not be admitted to The Community Center
before 12:00 pm, unless an adult accompanies them or
unless the child is enrolled into a program.
B. GUEST POLICY: All non-members must register and
guest fees must be paid before entering.
C. VALUABLES AND PERSONAL PROPERTY: Members are asked
not to bring valuables into The Community Center or on
its premises. Community Center management and employees
are not reliable for storage, safekeeping, or damage of
member’s property. The Community Center is not
responsible for any lost or stolen goods.
D. DAMAGES: The cost to repair damage to The
Community Center’s property by a member, member’s guest
and/or dependant children shall be paid by the
undersigned.
E. SUSPENSION OF MEMBERSHIP: The Community Center has
the right to suspend any membership due to non-payment
of dues, for violation of rules or policies, or for
behavior deemed inappropriate for the facility. |
